Immunizations and Child Care - What do I Need to Know?

Child care providers maintain documentation about immunizations and physical examinations as a licensing requirement. The immunization and physical examinations requirements are based on information from the Indiana Department of Health.
How do I prepare for child care enrollment?
Within 30 days of enrollment, you will be asked to provide documentation of immunizations and an annual physical exam. The physical exam must be less than one year old. It should include:
- Your child’s medical history.
- A physical examination.
- A written statement that your child does not have a health condition that would make participating in child care activities dangerous to your child or to other children.
- A written statement about special attention that your child may need in child care due to medical conditions or allergies.
The immunization record should show the month, day and year of each immunization.
What happens if my child cannot be immunized?
- If you choose not to immunize your child for religious or medical reasons, provide documentation to the child care provider.
- If your child is in the process of receiving immunizations, provide documentation from your child’s physician to the child care provider.
